Whether the syndrome of hyporeninemic hypoaldosteronism is reversible in some patient with lupus nephritis remains to be determined. We have present a case of reversible type IV RTA in a patient with lupus nephritis. Laboratory findings at
admission was
compatible with type IV RTA(normal anion gap, positive urine anion gap, hyperkalemia, metabllic acidosis, low PRA and plasma aldosterone). With the treatment of cyclophosphamide, metabolic acidosis was improbed and PRA and plasma aldosterone
level
returned to normal. This finding suggested that type IV RTA in lupus nephritis is reversible with the treatment of immunosuppresants, and this reversibility may be related to autoimmune process of lupus nephritis.
